Parental Consent, Release, and Liability Waiver for Minor Participants *

In consideration of participation in the AAPI Student Storytelling Workshop (“Workshop”) hosted by Make Noise Today a prgram under Creative Class Collective (a 501c3 organization) on February 22, 2025, I, the undersigned parent or legal guardian of the minor participant (“Participant”), hereby grant permission for the Participant to take part in the Workshop, which will be held at Intertrend - 228 E. Broadway, Long Beach, 90802 from 10:00 AM to 2:00 PM, with a 30-minute lunch break. Consent to Participate: I acknowledge that I have read and understand the event description and that I voluntarily consent to the Participant’s involvement in both the Writing/Literary Arts Workshop and the Visual and Performing Arts Workshop. I confirm that the Participant is physically and mentally capable of participating in the Workshop and that I have no objections to the Participant’s engagement in the planned activities. Assumption of Risk: I understand that participation in any educational, creative, or artistic event involves inherent risks, including but not limited to personal injury or property damage. I hereby assume all risks associated with the Participant’s attendance at the Workshop and agree that Make Noise Today, its officers, employees, agents, and volunteers are not liable for any injury, loss, or damage incurred, except where such liability is prohibited by law. Release of Liability and Indemnification: On behalf of the Participant and myself, I hereby release, waive, discharge, and covenant not to sue Make Noise Today, its affiliates, officers, directors, employees, and agents from any and all liability for any claims, losses, or damages arising out of or related to the Participant’s participation in the Workshop, including any claims arising from the negligence of any such parties. I further agree to indemnify and hold harmless Make Noise Today from any and all claims, liabilities, or expenses (including reasonable attorney fees) incurred as a result of the Participant’s participation. Consent to Photography, Video, and Audio Recording: I understand and agree that during the Workshop, the Participant may be photographed, videotaped, and/or recorded. I consent to the unrestricted use of such photographs, videos, and recordings by Make Noise Today for purposes including, but not limited to, promotional materials, social media, and archival documentation. This consent is given without compensation and extends indefinitely unless otherwise revoked in writing by the parent or legal guardian. I understand that such revocation will not affect materials that have already been published or distributed. Medical Treatment: In the event of an emergency, I authorize Make Noise Today and its designated representatives to secure, at my expense, any necessary medical treatment for the Participant. I agree to be responsible for any costs incurred as a result of such treatment. Acknowledgment of Understanding: I acknowledge that I have read this Parental Consent, Release, and Liability Waiver in its entirety and fully understand its terms. I understand that by signing below, I am giving up substantial legal rights, including the right to sue Make Noise Today and its representatives. I hereby certify that I am the parent or legal guardian of the Participant and have the legal authority to consent to the Participant’s participation in this event.
